Before you begin

Thinking about your usual patterns over the past six months. Respond about your own experience; there are no right answers.

This is an original SymptomScore self-reflection questionnaire, not a validated clinical instrument. Scores summarize your responses; they are not diagnostic cutoffs, probabilities, or population percentiles. Educational references describe the subject, not validation or endorsement of this questionnaire.

Your result saves to your profile on this browser. Answers stay in this tab while you complete the test and are removed from the draft when you finish. No account or cross-device sync is available.

For a repeat check-in, consider waiting 90 days. This is a reflection cadence, not a clinically validated retest interval.

How scoring works

Each response is scored from 0 to 4. Reverse-worded items are scored in the opposite direction. Dimension scores show the percentage of available response points. A score of 75 means three quarters of the response points, not a 75% chance of a condition.

The dimensions describe separate preferences or capacities. There is no overall total, fixed type, or best profile.
